| Shall perform a full range of Psychiatrist services on site, using government furnished facility, equipment, and supplies to a variety of patients. All aspects of
Psychiatric services shall be commensurate with the capabilities of the provider and the Medical Department including but not limited to the items listed below: 1. History, detailed examination,
testing and diagnosis; 2. Assess and treat outpatients in individual, group, couples, and family modalities exercising mature professional judgment and using a wide range of social work skills to
include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases/injuries; 3. Conduct psychosocial assessments to determine patient needs and
resources (both family support and community support). Provide counseling to patient and family in matters directly related to patients’ limitation, adjustment to medical condition, and ongoing
treatment. Develop and implement discharge plans, follow-up care, and transfers to other health care facilities (e.g. nursing homes, rehabilitation hospitals, and VA facilities); 4. Plan and maintain
referral and coordination services with civilian health and social service agencies to provide optimal patient care; 5. Provide consultation services to medical staff regarding psychosocial insures,
discharge plans, and follow-up care for patients and families; 6. Provide crisis intervention services, as directed; 7. Duty status recommendations in writing, to include duration, for all active
duty members; 8. Treatment plan recommendation and review; 9. Case consultation with referring Medical Department Representative; 10. Expert opinion in related medical board action; 11. Provide
immediate evaluation and management of emergent problems as they occur in addition to regular scheduled problems; 12. Prepare and submit appropriate documentation as specified in applicable
regulations; 13. If a medical emergency occurs to a patient who is receiving services from the Contractor, the Contractor shall immediately notify the Medical Department for assistance, and report
the nature of the emergency to the Commanding Officer or his authorized representative. |

| Shall posses and maintain the following: 1. A diploma from an accredited school of medicine 2. A current unrestricted license to practice medicine in one of the 50 States
3. A current board certification in the field of Psychiatry 4. A current Drug Enforcement Agency (DEA) registration certification 5. A “no unfavorable information on file” rating with the National
Practitioners Data Bank (NPDB) 6. A current certification in Basic Cardiac Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) by the American Red Cross or American Heart Association 7. A
minimum of two (2) years recent experience (immediately preceding the submission of this offer) |
